1. Submit this form prior to bringing your package to the Shipping Center.  The submission will be sent to both you and the Shipping Center, you do not need to print the form for us.  

2. Drop off your box with your name and the order number written on it.

3. Pay by phone or PayPal.  Once your package is received, shipping costs will be assessed.  A PayPal invoice will be sent to you for secure payment, or you may request we call you for credit card payment.  When payment has been received, your package will be shipped.
